When this patient arrived at our office he suffered from multiple cavities and broken down teeth. He wanted to know his options and did not want a denture! He suffers from a really bad gag reflex and knew that dentures were a horrible replacement for teeth. He had an implant placed previously and wanted to know more about replacing all his teeth with dental implants. We discussed removal of his existing teeth and replacing them with de­ntal implant bridges. This was the best option for him because of the following reasons:

Do you remember that Christmas song “All I want for Christmas is my two front teeth” ? Well that was the case for this patient. Due to an unfortunate accident this patient lost his two front teeth. As you can imagine this can be an embarrassing situation for anyone but fortunately with modern technology he was able to have his two front teeth for Christmas with two dental implants!

Fun fact! The front two teeth are the most fractured adult teeth during accidents!

Dr. Ozzie Rodas was able to replace this patient’s teeth with two front implants. As you can see the color match is near perfect. Dental implants are known as an artificial root and are normally made from a biocompatible materials such as, titanium or zirconia. This implant is then allowed to osteointegrate, this is a process by which your natural bone anchors the implant to the body.

After allowing the implant to heal for a period of 3-6 months digital molds are then taken to create the custom abutments (also known as a connector) and then the crowns that go over the implant as shown in the figure below.

When getting an implant many patients have questions such as:

  1. Will it hurt?
    • The short answer is No, our doctors are trained in techniques to provide local anesthesia with minimal discomfort. Ensuring that your experience is a pleasant one. Most patients say that they found the extraction of a tooth more painful than getting an implant placed.
  2. Will it look natural?
    • Yes, our doctor takes many photographs to communicate with the lab custom color to match your natural teeth.
  3. How successful is the procedure?
    • 99% successful, this success rate varies depending on multiple factors. There are health conditions such as osteoporosis, uncontrolled diabetes and low vitamin D that may increase implant loss. A proper consultation and physical examination is required prior to implant placements.
  4. How much will it cost?
    • There are multiple things to take into consideration to provide an accurate estimate such as: will you need a bone graft? If so is it a site preservation or ridge augmentation? These procedures vary in time commitment and financial commitment. If we are talking about just a single implant and crown well this price can vary from $3,500-6,500 in the Albuquerque, New Mexico area.

The success story! This patient had neglected their dental care because of their high dental fear. She was so scared of dental offices that it was hard for her to make it through the door let alone sit in the dental chair. Dental fear is one of the most common fears in the world! Fortunately here at High Desert Dental we office IV sedation for those patients that experience an extreme dental phobia.

This patient was able to restore years of neglect in a single visit while comfortably sedated. Her treatment included: scaling and root planing (AKA a deep cleaning) to get her gum health back in order, an implant to replace a missing tooth and porcelain crowns. We choose a color that closely matched her lower teeth. It was important for her to have a smile that “matched her personality” she did not want to have a “Hollywood fake smile”.

Implant Overdenture:

With the increase in dental implant awareness there can be several misconceptions about options out there and this success story involves an upper implant overdenture. This particular type of implant teeth snap into the implants. Shown below are the implant and the attachments slightly above the gums. Also you will see what the inside of the overdenture looks like, the blue attachments are very similar to a button for your jeans. These “buttons” or attachments snap into the implants are secured so that they do not move when you bite into things like apples.

The benefits of dental implants are:

  1. They help to maintain your bone level and decreases your risk of bone loss and therefore premature aging.
  2. Increased chewing capacity, a regular denture without implants decreases your chewing capacity by approximately 80%, have you seen a person with a denture eat an apple? Probably not. With dental implants you will regain your chewing force.
  3. Different retention caps: This is more of personal preference, some patients like their attachments to be so tight that they never move, but that makes it very difficult to remove which can be a good or a bad thing.
  4. They are removal, this is both a benefit and a draw back- some patients like the fact that these implant dentures are removable which makes cleaning the implants easier.
  5. Natural looking! These implant overdentures do look very natural and are definitely an upgrade from regular dentures.
  6. Costs effective, this procedure in general is a lower initial investment than an implant fixed prosthesis.

The drawbacks:

  1. It is removable as mentioned previously this can be seen as a benefit and a draw back. Some patients do not like the idea of having something that comes in and out because there is a stigma that goes with it. Also even though the retentive caps help to hold the overdenture in place it can sometimes pop out when eating really hard foods like almonds. It doesn’t happen too often but when it does it can be a little embarrassing.
  2. Extensions of the flanges, with an overdenture there are requirements for denture material thickness, though we can create a “horse shoe” style upper denture it still has some overhang to provide stability. This additional material may create a space for food impaction. Though one can easily remove the overdenture and clean them.

Implant All-on-4 also known as “All-on-X” “Teeth in a day” this success story is of a patient that had teeth that had several large cavities and keeping these teeth were not ideal for long term success and implants were the ideal treatment for this patient. Now, we know that there can be a lot of confusion about what the “All-on-X” procedure is and our goal is to help educate you on the different treatment modalities out there.

The primary definition of this procedure is a full set of teeth connected directly to 4-8 implants. In most cases the back implants are placed at an angle, this is done to avoid extensive and lengthy bone graft procedures and vital structures like your sinuses in the upper jaw and the inferior alveolar nerve in the lower jaw.

The complete set of teeth have small channels on the biting surface of the teeth and there are little screws that engage onto the implant platform that secures them to the implants. With this treatment you cannot take the teeth out but our dentists can. Now you may be wondering when would these teeth be taken out? Well they are taken out for cleanings every 6 months, the reason is because we have found that it is difficult to provide a thorough cleaning without removing these teeth.

The benefits of this treatment option are:

  1. Speed! Our office uses a complete digital process that allows us to provide our patients with a new smile in as little as 6 hours*** (time varies per patient)
  2. Superior material, the final implant teeth are made of Zirconia- this is not the case with all offices and it is important to know exactly what you will be getting. We have had patients arrived from other offices that have received a titanium bar with plastic denture teeth as shown below. The problem with this prosthesis is that the plastic teeth wear in 5-10 years and can break off the titanium bar. When this happens you will be left paying for these costly repairs. As opposed to what we provide is a solid zirconia set of teeth that look very natural and are so strong that you can run over the teeth with a truck! Though we do not recommend you try that at home.

As you can see by this patient’s success story these implant hybrid teeth look very natural and all our patients say they also feel very natural and provide them with a restored bite force to eat foods like hamburgers, steak, corn on the cob, pretzels and almonds just name a few of our patients favorite foods!

Dentures are for smiling, not for chewing” If you know you know that dentures are very difficult to eat with and have inherent draw backs such as:

  1. Bone loss, without teeth or implants to stimulate the bone there is an increase of bone loss in the upper and lower jaw. This bone loss is similar to the atrophy seen when a person has a broken arm in a cast. As bone withers away the dentures become more loose because the tissues supporting the denture also shrink. WIth additional bone loss there is increased aging.
  2. Low chewing force! Dentures only provide about 20% of your original chewing capacity, this makes it very difficult to proper masticate (chew) your food. This can leed to nutritional deficiency and digestive problems.
  3. Social stigma, unfortunately there is a certain stigma that is associated with dentures such as aging and not only that but recent studies have shown that patients that wear dentures have lower life expectancies.

This patient’s success story involves the All-on-4 also known as “teeth in a day“

Fluorosis! Fluorosis is a condition that can leave your teeth with unsightly stains and is caused when drinking water has too much fluoride. There are parts of New Mexico where this is an issue and it affected this patient’s smile. We were able to complete their success story with dental veneers and a single dental implant…Guess which tooth is the implant! In this case the patient wanted a more esthetic smile and we were able to achieve this through minimally invasive porcelain veneers. As you can see veneers can be made to look very natural and match the existing teeth and provide the patient with a functional smile
